This site is intended to help providers using Third-Party Electronic Visit Verification (EVV) vendors. For a successful integration onboarding with CareBridge, here are the 6 critical actions necessary that Providers and their Third-Party EVV Vendors need complete. 1. Complete Intake (EVV Vendor Action) • Third-Party EVV Vendors complete intake form: http://evvintegrationeform.carebridgehealth.com 2. Establish Third-Party Vendor EVV Credentials (EVV Vendor Action) • CareBridge creates or extends credentials for Third-Party EVV Vendor (Third-Party EVV Vendor sends public key in OpenSSH format or confirms that existing credentials from prior integrations can be extended) 3. Execute Testing (EVV Vendor Action) • Third-Party EVV Vendor completes testing with CareBridge (connectivity and data) 4. Request Provider Credentials & EVV Vendor Association (Provider Agency Action) can be complete at any point after #2 but must be complete prior to #5 • Provider agency completes access request form: http://evv.carebridgehealth.com/loginrequest • CareBridge support team completes access set-up and vendor association ‐ Sends necessary details to provider agency 5. Send Complete EVV Visits to CareBridge (Provider Agency & EVV Vendor Action) • Provider agency completes EVV visits using their Third-Party EVV system and sends visit data to CareBridge via Third-Party EVV Vendor for applicable health plans 6. Send EVV Claims (Provider Agency & EVV Vendor Action) • Provider agency sends EVV visit(s) with ClaimAction ‘N’ to CareBridge via Third-Party EVV Vendor for applicable health plan and the claim is fully adjudicated • Completed visits should be sent to CareBridge with no ClaimAction (“Null”) until prepared to bill the visit ‐ Providers must consult with their Third-Party EVV to manage this process
