CareBridge Provider Portal- Home Health Attending Provider Selection

19 STEPS

1. Click Settings

4. Scroll down to a desired payer and click the Settings drop-down

5. Scroll down and click here to use the Billing Provider when No Attending Provider is selected

6. Click here to always select the Attending Provider

7. Click SAVE CHANGES

8. To select an attending provider through an authorization, scroll up and click Authorizations

9. Locate the desired authorization, then click the Actions menu (three vertical dots)

10. Click Authorization Details on the pop-up menu

11. Click the Pencil icon to enable editing

12. Click the drop-down arrow under Attending Provider to choose an option from the drop-down menu or filter by typing in the Attending Provider NPI, first name, or last name

13. Click SAVE

14. To select Attending Provider on visits without an authorization, click Visits

15. Locate the desired visit, then click the Actions menu (three vertical dots)

16. Click Visit Details on the pop-up menu

17. Scroll down and click the pencil icon in the Billing card to enable editing

18. Scroll down to Attending Provider and choose an option from the attending provider drop-down menu or filter by typing in the Attending Provider NPI, first name, or last name

CareBridge Provider Portal- Home Health Admission Date Selection

16 STEPS

1. Click Settings

3. Click Home Health Phase 1

4. Scroll down to the desired payer and click the Settings drop-down arrow

5. Scroll down to the Admission Date section

Step 5 image

6. Click here if you would like to manage the Admission Date individually

7. Click here to align the Admission Date with the statement date

8. Click SAVE CHANGES

9. If you chose to manage the date individually, click Members

10. Find a desired member, and click their name

11. Scroll down to the Payer(s) section and click the "+" next to Certification Periods

12. Here you can set the Certification Period Start Date

13. And the Certification Period End Date

14. Alternatively, you can select the start sate then click a preset period of days

15. Click SAVE, and you're done!

CareBridge Provider Portal- Home Health Claim Frequency Selection

13 STEPS

1. Click Settings

2. Click Billing

3. Click Home Health Phase 1

4. Scroll down to the desired payer and click Settings

5. Scroll down to the Claim Frequency section and choose the left option to always send "1" for original claim submissions. (You can still choose a different frequency at the time of billing for initial / original claims)

6. Alternatively, you can choose the right option to specify claim frequency when exporting visits

7. Scroll down and click SAVE CHANGES

8. To manually set a claim frequency, scroll up and click Visits

9. Locate the desired visit(s) and click the check box next to it

10. Click EXPORT FOR BILLING

11. Click the drop-down arrow under Initial Frequency and select the desired frequency

12. To apply a single frequency to multiple visits, click the drop-down arrow under Bulk Set Frequency and select the desired frequency

13. Click EXPORT, and you're finished.

CareBridge Provider Portal- Home Health Condition Code Selection

17 STEPS

1. To set condition codes per authorization, click Authorizations

2. Locate the desired Authorization and click the Actions icon (three vertical dots)

3. Click Authorization Details

4. Click Condition Codes at the bottom of the details window.

5. Click +Add Condition Code

6. Click the drop-down arrow and choose a code

7. Click the Effective Date field and choose a start date from the calendar pop-up, then click ok

8. Click the Expiration Date field and choose an end date from the calendar pop-up then click ok

9. Click SAVE

10. To set condition codes per visit, click Visits

11. Locate the desired visit and click the Actions icon (three vertical dots)

12. Scroll down to the Billing card and click the pencil icon to enable editing

13. Scroll down to Condition Codes, click the drop-down arrow, then select a code

15. You can apply the condition code selection to the current visit only

16. Or you can apply the condition code selection for all visits with the same member and service that do not have a code already set (you can change this choice at any time)

CareBridge Provider Portal- Home Health Facility Type Selection

16 STEPS

1. The first step is to click Settings

4. Scroll down to the desired payer section and click the drop-down arrow under Settings

5. Scroll down to Facility Type and choose one of the options.

6. If the first option to use the same value is chosen, select the Value using the drop-down arrow

7. Or select the option to manage the facility type at the individual service provider level

8. Click configurable

9. Scroll down and click SAVE CHANGES

10. To manage facility type at the individual service provider level, click Provider Admin

11. Locate the desired provider and click Actions (three vertical dots)

13. Scroll down to the Facility Type section and click the pencil icon to enable editing

14. Click the drop-down arrow

CareBridge Provider Portal- Home Health Patient Status Selection

15 STEPS

1. Click Settings

4. Scroll down to the desired payer section and click Settings

5. Scroll down to patient status and click Always send 01 to send the default patient status (discharged to home or selfcare)

6. Or click Manage status on an individual basis to manually set patient statuses

7. Click SAVE CHANGES

8. To manually set a patient status, click Members

9. Locate the desired member, and click the member's name

10. Scroll down to the payer section and click the "+" next to Patient Statuses

11. Click the Status drop-down arrow

12. Select a status code from the list

13. Click the Effective Date field

14. Select the desired date, then press OK
