The purpose of this document is to provide step-by-step instructions for downloading and logging into the CareBridge Mobile Application. This covers Apple iOS and Android OS smartphones.

The CareBridge mobile application is available to Caregivers and can be downloaded from the App Store or the Google Play store. If you need help with downloading the app, please refer to the Mobile Application Download and Login Guide in the CareBridge Resource Library. The instructions below will tell you how to Check-In and Check-Out of appointments within the mobile application.

If you are unable to use the CareBridge Mobile Application, you can use the Interactive Voice Response (IVR) option to Check-In and Check-Out of a visit. This method takes more time to complete and you will need to use the Member’s home phone to call the IVR phone number. You will need three different codes that must be entered each time you call in using IVR. The three codes you will need are your Provider ID, IVR PIN, and IVR Passcode. Your Provider ID is 292, your IVR PIN will be the same number as your Username, which is your Veridian Employee ID number. You will also need your 8-digit IVR Passcode, which you create the first time you use IVR. You will need all three of these to use the IVR system.

A manual entry must be completed if you need to fix or add visit information. Manual entries can be used to fix or add dates of service, Check-In, and Check-Out times. If you forgot to Check-In and Check-Out of a visit you will have to create the visit when you remember, then follow the steps below to make the needed corrections.
